Fill out each box to the best of your knowledge. You may not know the numbers at the start of this assessment. One goal of this assessment is to help you identify how you can measure each decision level.
Column B: Actions You Are Taking
In column B, list actions you are taking, or should be taking. A check-mark indicates you are taking this action. A box indicates that you are not yet taking that action.
